Elevator Request Form
This Elevator Request Form is required for all move-ins, move-outs, and deliveries/ pick-ups managed by professionals or individuals.
General Rules:
This reservation request form does not represent any exclusivity for use of the elevators. Other residents may have access to the elevator(s) during reservation hours.
2. Deliveries, move-in, and move-out are Monday to Friday, 9:00 AM to 4:00 PM. No weekends or Holidays.
3. All bookings must be made 48 hours in advance with a COI (certificate of insurance)and a deposit.
4. All items must be ready to load/ unload via the garage entrances. Deliveries are not permitted from the building’s main entrance (lobby).
5. All moving/ delivery companies are required to submit a Certificate of Liability Insurance. The certificate holder should be listed as follows: Bay Harbor Club Association, Inc.
6. All common areas (elevators & hallways) must be protected with corner guards, walls, and carpet protection.
7. All cardboard boxes must be collapsed and discarded on the ground floor/ refuse room. Hallways may not be used for storage. Trash chutes may not be used to discard boxes or other moving/ delivery refuse.
Fees:
A $500 refundable security deposit is required to schedule a move-in/out or delivery.
Payable only by money order or cashier's check. Checks must be made payable to: Bay Harbor Club Association, Inc.
Payment must be received 48 hours in advance.
Violations:
Owners and Residents who violate these procedures and any rules and regulations set forth in the condominium documents may be subject to a fine of $100 to $1,000.
